Admission Process:

Step 1 -

Call for an appointment to come visit us! If you have family members who can’t come to the tour we would be happy to mail them a brochure that outlines all of our programs and amenities.

Step 2 -

Come take a tour! You may choose to bring your loved one with you or you may come first yourself to look over the campus.

Step 3 -

Request a free comprehensive pre-assessment for your loved one. We will meet with you and your loved one personally and confidentially. This visit allows us to determine where each resident is best placed to meet his or her social, physical, medical condition and needs.

Step 4 -

We will then provide you with an exact dollar quote of the rate per month. No hidden fees or small print. At this time you will decide if you would like to move ahead with placement. You may request a room hold if you are not quite ready to move in. Our facility is very popular, and admission is not always automatic. If a room is not available you may request to be put on our waiting list free of charge.

Step 5 -

When it is time for your loved one to move in we will need to have the admissions packet completed with the resident or POA/guardian. A TB screening and physical from your physician is required within 90 days prior to admission, or for emergency placement we will require the information within the first week.

Step 6 -

On the day you move in, you are welcome to bring your family members' own furnishings: however all our rooms do come fully furnished at no additional cost. Please mark clothing with a permanent fabric marker. We will complete an inventory sheet so we have a record of what is brought into the facility.

Step 7 -

You are always welcome to visit at Hilltop. Stay in touch. We welcome your feedback! Let us know how we are doing. We enjoy talking with you and learning more about your loved one, now part of our family as well!
